INJURY REPORTING (WORK-RELATED)

All work-related injuries or illnesses must be reported to PCSU immediately (within 24 hours)!

Please submit injury reports to Kahale Pali (PCSU Safety Coordinator) at pali@hawaii.edu and cc. Your Principal Investigator. Injury Reports must be submitted to PCSU for Principal Investigator review and signature before it goes to RCUH.

Vehicle Accident Reporting

Notify PCSU immediately of any vehicle accidents.
Send reports to Linda Hara (hara@hawaii.edu), and cc. your Principal Investigator.
